Healthwatch Stockton-on-Tees Hospital Experience and Discharge Report
A new hospital discharge process was introduced nationally early in 2020 to support the NHS and its response to the coronavirus pandemic. Following our 'Hospital Experience and Discharge' survey, which was conducted from March - November 2020, our report explores the views and experiences of people who have been in hospital and how the discharge process affected them.

Alcohol Services Follow Up - July 2018
Healthwatch Stockton-on-Tees carried out an investigation in September 2017 to research public awareness and experiences of local alcohol services, alcohol related services (such as advice/counselling), alcohol-related hospital admissions and A&E attendances. Following this piece of work, Healthwatch Stockton made a number of recommendations to Public Health to help improve service user experience of alcohol services. The aim of this follow up is to identify if the recommendations have been actioned and if service user experience has improved as a result.

University Hospital of North Tees Communication Follow Up - June 2018
Healthwatch Stockton-on-Tees carried out an investigation at the University Hospital of North Tees in January 2017. This investigation was carried out following a number of pieces of work completed by Healthwatch Stockton-on-Tees in the University Hospital of North Tees, (between 2015 and 2016) where patients had reported that communication could be improved. The purpose of the follow-up visit was to identify if any changes had been made as a result of Healthwatch Stockton-on-Tees’ recommendations and to identify if patient and staff experience has improved.

Enter and View: Arrival Practice - June 2018
Healthwatch Stockton-on-Tees attended Arrival Practice’s Patient Participation Group (PPG) to give a presentation about the role of Healthwatch and to gather feedback from the attendees. Healthwatch informed the PPG about the statutory powers it has and gave examples of some of the work which has been carried out over the past year. Healthwatch Stockton-on-Tees received an overwhelming amount of positive feedback and praise for the practice and its staff. Following this meeting, Healthwatch approached the Practice Manager and arranged to carry out an Enter and View visit to gather more patient and staff feedback with the aim of sharing this as good practice.

Yarm Medical Practice Enter and View - May 2018
Healthwatch Stockton-on-Tees attended Yarm Medical Centre’s Patient Participation Group (PPG) to give a presentation about the role of Healthwatch and to gather feedback from the attendees. Healthwatch informed the PPG about the statutory powers it has and gave examples of some of the work which has been carried out over the past year. Following this meeting, the Practice Manager invited Healthwatch to the practice to carry out an Enter and View visit to gather patient feedback and identify any areas which may require improvement. Provision of Services for Adults with Learning Disabilities - April 2018
Stockton-on-Tees Borough Council have been running a day option service for adults with learning disabilities which commenced five years ago. Changes to the provision of these services have occurred over the last two years following an internal review.
Healthwatch Stockton-on-Tees liaised with Stockton-on-Tees Borough Council after receiving public feedback around these changes to the provision for adults with learning disabilities. Healthwatch were keen to find out what current provision had been put in place.
Stockton-on-Tees Borough Council welcomed the opportunity for Healthwatch to visit their newly refurbished Day Options facilities at Allensway Centre and Stockton Business Centre which delivers community-based learning disability day services to support adults with learning disabilities in the community.

Report on patient experience of dental services in Stockton-on-Tees - February 2018
Public Health informed Healthwatch Stockton-on-Tees that Oral Health was one of its future priorities for 2018/2019, and welcomed any feedback Healthwatch Stockton-on-Tees could gather from patients by carrying out a planned engagement activity. Healthwatch Stockton-on-Tees asked all dental providers in the locality to take part in the engagement activity. The staff team and volunteers talked to patients at each of the 16 dental practices visited using a survey to gather feedback. Please see the report below.

Enter & View: Elm Tree Surgery - November 2017
Healthwatch Stockton-on-Tees (HWS) received an overwhelming volume of feedback regarding access to, and service provision of, primary care services in the area throughout 2016. This feedback contained comments, concerns and praise for a number of GP surgeries in Stockton-on-Tees and informed the GP analysis report HWS published in October 2016. Feedback from the public continues to be captured by HWS. The positive responses, and evidence to suggest some practices are delivering a quality service, led to HWS conducting an Enter and View at Elm Tree Surgery, to gather further evidence of this and share best practice with other providers in the locality.

Roseberry Practice Follow-up Report - November 2017
Healthwatch Stockton-on-Tees carried out an Enter & View visit at Roseberry Practice in February 2017. The visit was as a consequence of Healthwatch Stockton-on-Tees receiving a number of complaints about the practice having no male GP’s, and the difficulty patients were experiencing accessing the practice by telephone. Patient and staff feedback was gathered during the February Enter & View and collated in to a report which included recommendations for change with the aim of improving patient experience. A response was not received from the provider during the 20 day response period or following several follow up requests.
The purpose of the November 2017 follow-up visit was to observe if any changes had been made as a result of Healthwatch Stockton-on-Tees’ recommendations. To read the follow up report please click on the attachment below.

Enter & View: Implementation of John's Campaign and support for patients living with dementia at University Hospital of North Tees - October 2017
HWS are part of the North of Tees Dementia Collaborative network, and work closely with the Live Well Dementia Hub and Dementia Voices Stockton. As a result of this close partnership working, feedback suggested that improvements could be made to improve the experience of patients living with dementia when in a hospital setting. John’s Campaign was founded in November 2014, with the aim of encouraging Trusts, to make a commitment to enable family and carers of patients living with dementia, to remain with them in the hospital setting, outside of visiting hours; also to support with care needs. To date, 1143 establishments have signed up to the campaign, with most Trusts across England making the commitment. The founder of John’s Campaign spoke to HWS and informed them that the University Hospital of North Tees remains one of only 15 that have not. Its successful roll-out in James Cook University Hospital has been acknowledged by many.
Healthwatch Stockton-on-Tees conducted and Enter & View to investigate the support for patients living with dementia, their family and carers in the University Hospital of North Tees.

Analysis of Continence Service in Teesside - September 2017
Healthwatch works closely with both the voluntary and public sectors to assist in the delivery of its statutory activities and priorities. These close relationships ensure the public voice is heard during the service planning processes, making sure local services better meet the needs of the people using them.
Through engagement with the LiveWell Dementia Hub and Stockton Dementia Advocacy Service, and individuals who use the service in Stockton, it was brought to the attention of Healthwatch that the continence service provision is not meeting the needs of service users. Local Authority representatives told Healthwatch that they also felt that information gathered could inform change as issues about the service provision are frequently brought to their attention.

Alcohol Services Report - September 2017
Healthwatch Stockton-on-Tees works closely with both the voluntary and public sectors to assist in the delivery of its statutory activities and priorities. These close relationships ensure that the public voice is heard during the service planning, making sure local services better meet the needs of the people using them. One of the statutory bodies Healthwatch work closely with is Public Health Stockton-on-Tees Borough Council (Public Health). Public Health are the lead body for developing and publishing our local Joint Strategic Needs Assessment (JSNA). The JSNA identifies and summerises, defines and addresses the wider health and well-being needs of the people of Stockton-on-Tees.
Work done in partnership, identifies and gathers intelligence needed to ensure commissioning strategies work together for better provision of services for those areas of greatest need. For more information on JSNA, please follow this link http://www.teesjsna.org.uk/stockton/

Feedback on the Integrated Urgent Care Service at University Hospital of North Tees - July 2017
The new Integrated Urgent Care Service was launched on 1st April 2017 with Centre’s located at the University Hospital of North Tees and the University Hospital of Hartlepool. As an NHS service it is funded by the Clinical Commissioning Group.
Healthwatch Stockton-on-Tees visited the University Hospital of North Tees Integrated Urgent Care facility to gather feedback on how service users found their experience.

Podiatry Services Engagement Event - May 2017
NHS Hartlepool and Stockton-on-Tees Clinical Commissioning Group (HaST CCG) are engaging with the public regarding the current provision of NHS podiatry services in Hartlepool and Stockton-on-Tees. The intelligence gathered will be used to help shape and influence changes to the service to better meet the needs of those using the clinics. HaST CCG approached Healthwatch Stockton-on-Tees as a critical friend with a proven track record of public engagement to support with the engagement and consultation.

Following work completed by Healthwatch Stockton in 2014 ‘Arrangements for Discharge’ from the University Hospital of North Tees, further work was carried out investigating care package arrangements following patient discharge in February 2016.
A follow up visit was arranged and conducted in December 2016 by Healthwatch Stockton-on-Tees and welcomed by the Trust. Autism Spectrum Disorder (ASD) and Dental Service Provision in Stockton-on-Tees - December 2016
During recent engagement activities, a number of individuals and professionals working within the Voluntary, Community and Social Enterprise Sector (VCSE) have fed back concerns with the accessibility of, and care provided by the dentists for people diagnosed with Autism Spectrum Disorder (ASD) in Stockton-on-Tees.
To gather further intelligence from local residents about this issue, Healthwatch Stockton-on-Tees designed a questionnaire and worked in partnership with other organisations including Daisy Chain and Stockton United For Change (SUFC) to distribute it and collect responses to find out if this was a wider problem.

GP Data Analysis Report - October 2016
Since January 2016, Healthwatch Stockton-on-Tees have received an overwhelming volume of feedback regarding access to and service provision of primary care services in the area. This feedback contains comments, concerns and praise for a pnumber of GP surgeries in Stockton-on-Tees. Healthwatch have collated this feedback into a report with some recommendations to address current problems faced by patients at these practices. Read the report here.
NHS 111 Survey Feedback - October 2016
A representative from Healthwatch Stockton-on-Tees regularly attends the North East Ambulance Service Healthwatch Forum. At a previous meeting, Healthwatch attendees were informed that the next meeting would focus on the 111 service and that this would involve a discussion and opportunity for Healthwatch representatives to feedback on the views of the residents in the local areas.
Healthwatch Middlesbrough, Stockton-on-Tees and Redcar and Cleveland worked together to gather this feedback from the local communities in order to give a true representation of the resident’s views, at this proposed meeting. Read the report here.
Enter & View: Woodside Grange Care Home - September 2016
Through our information and signposting service and public engagement, Healthwatch Stockton-on-Tees received intelligence from members of the public concerning the service delivery at Woodside Grange Care Home, Tedder Avenue, Thornaby.
From the information received, the Healthwatch Stockton-on-Tees Executive Board made the decision to conduct an Enter and View visit at the Care Home. The purpose of this visit was to conduct a more in depth examination of how the service is delivered, to listen to the views and experiences of members of staff, residents, family and friends, with the aim of providing recommendations from the findings to help improve service delivery and ensure the needs of the residents are met.

Delays to Patient Discharge Follow-up Report - September 2016
Following work completed by Healthwatch Stockton in 2014 ‘Arrangements for Discharge’ from the University Hospital of North Tees, further work was carried out investigating transport arrangements following patient discharge in February 2016.
A follow up visit was arranged and conducted on 6th September 2016 by Healthwatch Stockton-on-Tees and welcomed by the Trust. The purpose of the follow up visit was to assess what actions had been made following Healthwatch’s recommendations to improve patient experience during discharge from the discharge lounge. Investigating Patient Experiences of Improving Access to Psychological Therapies (IAPT) - August 2016
Following Healthwatch’s ‘Shaping Mental Health Services Together’ event, a range of comments and concerns were received from those who attended regarding current mental health service provision.
Patients/carers expressed long waiting times to access services which was found to be a major concern. There is the feeling that there are gaps between services which need bridging with a sharing of information with regards to what’s available and where the services can be found, in particular with GP’s as this is usually a patient’s first point of contact. In addition to this, it was felt that some doctors are not listening to their patients and find it is ‘too easy to just give medication’. There is a need to improve inter-departmental communication to prevent patients having to repeat ‘their story’ more than once.
It was felt that there is no awareness and little promotion of mental health services for those in the community, which should not just be targeted to those who have a mental health condition. There appears to be a lack of choice and information available with barriers to accessing services e.g. the language used. Concerns were expresses by those who have accessed mental health services in the three localaties regarding the lack of prevention services due to limited resources. Barriers to accessing services for patients who have sensory or learning disabilities was also highlighted.
Following this feedback, Healthwatch decided to carry out further research into these issues.
Healthwatch are aware that the feedback received from this investigation was only from a small proportion of residents in Stockton, Middlesbrough and Redcar & Cleveland which is not a representative sample and that the individual needs and availability of services varies between the three areas. However, Healthwatch felt that it was important to share this information with the commissioners, providers and wider public. Reasons for this are that individuals who Healthwatch spoke to echoed similar experiences in relation to mental health services they had used regardless of where they had accessed these services. This small snapshot helps to identify where potential improvements or further research should be carried out to improve these services in the future.

Shaping Mental Health Services Together Event - March 2016
Healthwatch Stockton-on-Tees, Healthwatch Middlesbrough and Healthwatch Redcar and Cleveland hosted an event in collaboration with North East Together and Voices for Choices to gather views and experiences of those who access mental health services in the Tees locality.
The event attracted over 120 people including service users, carers, service providers and staff from the local Clinical Commissioning Groups(CCG) and NHS England.
Attendees provided valuable information regarding issues or concerns and made suggestions for improvements to consider for the future development of mental health services.

University Hospital of North Tees Survey - February 2016
Following public engagement, Healthwatch Stockton-on-Tees received information regarding service provision at the University Hospital of North Tees. An online questionnaire was generated to gather further information about patient’s views and experiences of accessing services at the University Hospital of North Tees. The questionnaire was publicised on Healthwatch Stockton-on-Tees’ website, Facebook, Twitter and Streetlife pages to ensure a wide range of data was collected.

Enter and View: Thornaby and Barwick Medical Group - January 2016
Through our information and signposting service and public engagement, Healthwatch Stockton-on-Tees recieved intelligence from members of the public concerning the service delivery at Thornaby and Barwick Medical Group.

Public Engagement Summary: Sexual Health Services - May 2015
Starting in 2014, the Tees Public Health Shared Service (TPHSS) began a full review of sexual health services provided in the Tees Valley, in preparation for the recomissioning of the services.
Healthwatch Stockton-on-Tees decided to undertake an independent review of users services views and experiences of current sexual health services provided to Stockton-on-Tees residents to ensure their voices were heard.
Healthwatch endeavoured to determine what is currently working effectively within service delivery, how the service could be improved and how the service could better suit service users needs in the future to inform the design of the services within the recommissioning process.

During engagement activities, some people questioned the inconsistency of planned activities for residents at Roseville Care Centre, situated in Ingelby Barwick. The Healthwatch Stockton-on-Tees Board decided to undertake an Enter and View at the home to investigate the concerns raised by consulting with staff, residents and their relatives. The aim of the Enter and View was to assess the activities available to residents, the accessibility of activities and determine if they meet the needs of residents.

Independent Consultation on Stockton Borough Councils Review of Homecare Services - February 2015
Healthwatch Stockton-on-Tees carried out this piece of work in partnership with Stockton Borough Council during the recomissioning process of Homecare Services in Stockton-on-Tees, which begun in 2014. Healthwatch aimed to provide the users of Home Care services with the opportunity to express their views on how their Home Care is delivered to an independent body. A cohort of service users completed surveys and a focus group was undertaken with young carers, whom the individual they care for recieve Home Care Services, to gather their views and experiences. The findings will inform the future design and commissioning intentions of future Home Care services provided in the locality.

University Hospital of North Tees - Arrangements for Discharge from Hospital - October 2014
Inteligence gathered from patients of the University Hospital of North Tees through public engagement and consultation activities highlighted that patient experience during the discharge process was a cause for concern. The Healthwatch Board decided to direct study into exploring this issue in more detail and examine whether the current service meets the needs of patients, families and carers.

Eye Health Needs Assessment: Patient and Public Engagement Summary - 2013
The North East and Cumbria Local Eye Health Network decided to incorporate an Eye Health Needs Assessment (EHNA) into its work priorities for 2013/14- 14/15 for the Durham, Darlington and Tees area.
The Eye Health Needs Assessment aims to to gather a strong strategic evidence base to inform future commissioning intentions of eye health services based on the needs of residents living in the area, ensuring a true representation of the eye health needs is given and informs the design of service delivery. The intention of the assessment is to contribute to long term goals of reducing health inequalities experienced by individuals with poor eye health, improve service provision and service access for service users.
Healthwatch Stockton-on-Tees undertook the patient and public engagement for the Eye Health Needs Assessment covering the Stockton-on-Tees area. This has allowed Healthwatch to provide an independent perspective on local eye health needs, ensuring that a voice is given to seldom heard communities and 'at risk' groups. The data collected from patients and the public will be fedback to support future eye care commissioning decisions amongst various service commissioners and providers.

Stockton Borough Council's Adult Mental Health Services Review: An independent Consultation with Service Users and Carers Affected - 2013
As part of our first piece of work, Healthwatch Stockton- on-Tees has produced an independent response to the Local Authority’s Review of Adult Mental Health Services.
Working with our delivery partner Middlesbrough and Stockton Mind, we have collected the views of service users and carers who will be most affected by the proposed changes.
Some of the key themes of the report included personal budgets, recovery, alternative provision and involving service users and carers in decision making.
